Our mission is to help you attain your goals, including relief of pain and improved physical functioning.

Silk Physical Therapy Center will submit claims to your insurer for treatments and services performed by our facility. We recommend that you become familiar with your plan so you are aware of your financial responsibilities. We accept patients with almost all plans. Please contact your insurance plan directly to confirm that we are considered a Participating Provider in your network.
Payment not covered by your insurance is due at the time of service. If this is not possible, we are happy to discuss making a payment plan for you. It is the patient’s responsibility to be aware of insurance plan deductibles, co-payments, and co-insurance.
